Longitudinal follow-up means seeing how a patient changes between sessions and over the whole course of therapy. Theramate builds it with two tools. Psychometric questionnaires measure, self-observations capture daily life. Every response comes back to the file, dated, scored and comparable with the previous one.
Two tools, one follow-up
| Psychometric questionnaires | Self-observations | |
|---|---|---|
| Purpose | Measure | Capture daily life |
| Content | Catalogue or imported scales | Tailor-made form |
| Frequency | One administration, then another | As often as agreed |
| Result | Score, level, subscales | Responses and summary |
| Over time | Curve and change | Dated history |
Both live in the same place, in « Clinical tools », under « Patient tools ». The patient responds from their phone, and you never copy out a score.

Administering a questionnaire in session or remotely
- « Fill with patient » for an administration in session, on your computer or your iPad.
- « Share with patient » for a remote administration, by email or with a link to copy.
The patient opens the link without an account. The page adapts to the phone, a bar shows progress and any skipped questions are flagged before sending.
The questionnaire can also come from the session. If you note “repeat the PCL-5 in a month”, Theramate turns it into a task with a « Send » button. Nothing goes out without your approval. Tasks are described in Everything Theramate prepares when you close a session.
Scoring, automatic
As soon as the patient responds, Theramate calculates the score. Reverse-scored items, subscales, cut-offs and screening rules are applied for each instrument. When the scale provides for it, as with the AUDIT-C, the interpretation takes sex and age into account.
You receive a « Questionnaire completed » notification, which shows neither the patient’s name nor the name of the scale. Each response gives you three views.
- « Interpretation », with the subscales and the scores by domain.
- « View responses », item by item.
- « Norms », which opens the scale’s scoring guide.
Progress, administration after administration
Responses are grouped by questionnaire, because it is the scale that is compared over time. A curve plots the score of each administration, the change since the previous one is shown alongside, and each administration keeps its date on a timeline. A PCL-5 at intake, another at three months, a third before the end of therapy, and progress can be read at a glance.
Import your own questionnaires
A licensed scale, one from your training or one built by your team comes into Theramate in a few minutes.
| Step | What happens |
|---|---|
| Document | You upload a PDF, a Word file, an image, an Excel file or a text |
| Methodology | Theramate extracts the questions, the answers, the reverse-scored items, the subscales and the levels |
| Clarifications | You check the preview, as the patient will see it |
| Saving | You confirm that you hold the necessary rights |
The imported questionnaire then works like those in the catalogue. It is shared by link, scored automatically and tracked over time. You can correct it whenever you like with « Edit ».
Self-observations, tailor-made
A self-observation is a form the patient fills in between sessions. You create it with « New self-observation », describing what you want to track, as you would to a colleague. The assistant writes a short introduction for the patient and chooses the fields.
| Approach or situation | Self-observation | What the patient notes |
|---|---|---|
| CBT | Automatic thoughts | Situation, thought, emotion, intensity |
| Exposure | Exposure tracking | Situation, anxiety before and after |
| Sleep | Sleep diary | Bedtime, awakenings, quality |
| Addictions | Cravings | Moment, trigger, response |
| Couples | Moments of tension | Situation, what helped calm things |
The available fields are free text, slider scale, checkbox, multiple choice with an « Other » answer, date and time. To adjust the form, you ask the assistant (“add the duration of the episode”), and the rest of the form stays as it is.
When a task from the session calls for a diary, Theramate prepares the draft in the background and lets you know with « Self-observation ready ».
Sharing, in a few settings
| Setting | Questionnaire | Self-observation |
|---|---|---|
| Patient account | None | None |
| Link validity | 30 days, adjustable | 7, 30 or 90 days |
| Number of responses | 1, adjustable | Unlimited or capped |
| Access code | 4 digits, optional | 4 digits, optional |
| Personal message | Yes | Yes |
For discretion, the email never shows the title of the self-observation. The patient responds in their own language, rereads what they sent with « View my previous responses » and keeps it with « Export as PDF ».
Every response, summarised and dated
With each submission, Theramate writes a « Response summary », short and faithful to what the patient wrote, without interpretation. A blue dot marks unread responses. You browse the current month or the whole history.
In session, what is new
| Questionnaire | Score | Level | Received |
|---|---|---|---|
| GAD-7 | 9 | Mild | 14 Oct |
| PSS-10 | 22 | Moderate | 14 Oct |
During preparation and during the session, the « Tools & Reports » panel brings together everything that has come in. Self-observation responses are split into « Since the consultation of … » and « Earlier ». You start the session knowing what the patient has been through since last time.
A follow-up your assistant and your reports can read
- The intervision assistant reads the scores with their dates and their trend, for example a PCL-5 that went from 52 to 31. You can ask it how the patient has progressed since the start of therapy.
- Clinical reports, such as the « Therapeutic evolution review » or a letter to the GP, draw on the actual scores and on the patient’s responses.
- The file export includes every administration, the scores, the responses and the self-observations.
Giving the follow-up back to the patient
From the responses to a self-observation, « Prepare a report » writes a « Your observations » document for the patient. It suggests avenues to explore together. You review it, then « Preview and share » sends it by email protected by the patient code, prints it or downloads it.
Protected responses
- Responses received by link are encrypted with a key unique to each practitioner.
- Each link is unique, time-limited and can be deactivated.
- Notifications stay neutral.
- Your file is hosted and operated in the European Union, on HDS v2.0 certified infrastructure.
The details are on the security page. The patient follow-up page presents all the tools for follow-up between sessions.
The criteria for choosing practice software are gathered in How to choose software for your therapy practice in 2026.
- 60 psychometric questionnaires across 21 areas, scored automatically with their subscales.
- Your own questionnaires imported from a PDF, a Word file, an image or an Excel file, and scored like the others.
- Self-observations created by describing what you want to track, filled in by the patient on their phone.
- Every response dated, summarised and filed, with the change since the previous administration.
- In session, you see at a glance what has come in since the last appointment.
Frequently asked questions
Does the patient need an account to respond? ▾
No. They receive a link by email and respond from their phone or computer, without an account. You can protect the link with a 4-digit code.
How many psychometric questionnaires are available? ▾
60 questionnaires in English, across 21 areas, from depression and anxiety to trauma, addictions, couples and childhood. You can add your own questionnaires.
Are the questionnaires scored automatically? ▾
Yes. Theramate calculates the total score, the subscales and the level, taking reverse-scored items into account. You also see the change since the previous administration.
Can I import a questionnaire I already use? ▾
Yes. Import the document, Theramate extracts the questions, the answers and the scoring, you check the preview and confirm that you hold the necessary rights. The questionnaire is then shared and scored like those in the catalogue.
How do I create a tailor-made self-observation? ▾
With « New self-observation », you describe what the patient should note. The assistant builds the form, you adjust it by asking for changes, then you share it by link.
